MEMBERSHIPS INCLUDE:

10% Off any other services, upgrades, and product purchases

*Refill Credits are based on billing cycle. Membership dues pull the membership amount and apply to the accout as two credits every 30 days.



OUT OF POCKET PAYMENTS

2-3 times out of the duration of the membership a member will have to pay out of pocker for their service(s) due to differing days in a month. Members will also need to pay out of pocket for service(s) at the member discounted rate when they redeem and/or use all acitve credits before their next auto draft.


CREDIT ROLLOVER

Unused membership credit may rollover from month to month, as long as your membership is active and in good standing. Three accumulated credits may be redeemed towards a full set less than or equal to your membership level with a Regular Stylist.


MEMBERSHIP FEES

Your membership dues will be due on the same day each month ( your autopay pay date) thereafter until your membership is canelled. You must have a valid credit card on file, and it will be charged the amount of your membership on your autopay date. A vakud credut card and email address is REQUIRED for all memberships.


PERSONAL USE OF SERVICES

Your membership is yours and yours only, by signing up you are agreeing that you are purchasing this membership for your own personal use. Your Membership is non-transferable, and you cannot gift the credits or benefits under your membership to any other individual.


APPOINTMENT CANCELATION

Appointments must be cancelled atleast 24 hours in advance, otherwise the appointment will be considered to have been used and an available pre-paid membership credit will be redeemed from your account. If you do not have an available credit in your account, you will be charged a cancellation fee of the entire cost of service(s) booked for that appoinment.If you arrive late to an appointment, you may not be entitled to the full appointment time and the stylist may end the appointment at the allotted time. If you fail to arrive at all for your appointment, you will forfeit the service.


SALON AND SERVICES

The Salon cannot gaurantee that your preferred stylist will be available, nor that ther ewill be sufficent availability with respect to a certain level of stylist at times you prefer.The Salon may be closed from time to time due to weather, holidays,repairs, and other routine and non-routine maintenance and miscellaneous matters.


MEMBERSHIP FREEZE

You have the right to freeze your membership, retaining your membership fees rate, ONCE, during the life of your membership for up to three consecutive months. You must provide 2 buisness days written notice before the freeze is to take effect. A membership cannot be canceled during the term it has been frozen. If cancellation notice is received during the freeze period, the thirday (30) Days written cancellation notice will be considered to start upon the freeze term expiration date. A membership freeze form is available at any Lash Lounge Salon and must be filled out and returned to your home salon.

MEMBERSHIP CANCELLATION PROCESS

THE MINIMUM TERM OF YOUR MEMBERSHIP IS ONE MONTH. After the minimum term, your membership will be automatically renewed for the successive one-month periods unless and until your membership is cancelled. You may cancel your membership at any time following the initial one-month term, with at least thirty (30) days written notice. The cancellation notice form is available at any participating Lash Lounge salon and must be returned to your Home Salon. Cancellation of your membersip will terminate this agreement and you will no longer be entitled to use benefits of the membership, including redemption of unredeemed credits and rollover credits. Any unused membership credits accrued during your active membership period(s) may be redeemed within the 60 days from the date you deliver a cancellation notice to your home salon, or the date you receive your salons notice to cancel your membership. After this 60 day period, all undused membership credits accrued will be forefeited and non-redeemable.


SALON'S RIGHTS TO CANCEL

Your Home Salon may cancel this agreement upon 30 days written notice to you--notice of which will b e made by mail or email at the addresses listed on your profile. Notice will be deemed as received by you three days after the date mailed or the day a cancellation email is sent. Payments due prior to the effective date of termination will be charged as scheduled. The termination will be effective 30 days after the date the letter was deemed as received.


RECIPROCITY

You may redeem the Membership services at any Lash Lounge Location, however, the Membership credit rates may vary by location. Additional fees may apply at locations that charge a higher rate for services than your home salon.


SALONS RIGHT TO NOTIFY

Home Salon reserves the right to modify the terms and conditions of the membership program upon 30 days written notice to you -- notice of which will be made by mail or email at the addresses listed on your profile.


PERSONAL EMAIL ADDRESS

By providing your email address you may recive occasional emails from Home Salon updating you on memberhship terms or other news. By signing below, you agree that Home Salon may use your email to provide a formal notice with respect to the terms, including cancellation or termination of this agreemment and you consent to the use of your email for this purpose.
